Boxes need grayed out default text
By default have "Normal Day / Shift" checked
"Normal Day / Shift" and "Call Out" are exclusive
Change both "Normal Day / Shift" and "Call Out" to be radio buttons
"Pay Point 2" will not be visible unless the user selects a "Pay Point 1" that has children fields
Mode of transport needs to be added (radio buttons)
